Welcome to the plan!
Use the Pension Enrolment Card and Beneficiary Designation form to enrol in the
Ironworkers Local 97 Pension Plan and to select your beneficiaries. Once a Member
of the Plan, all changes to Member information or beneficiaries should also be
made by completing this form in full.

Information Needed to Complete the Form
Your personal Member information is needed to complete the form.
Please follow the instructions on the form and ensure that you have completed the card in full.
If you have a spouse, you must designate him/her as your Pension beneficiary.
If you do not have a spouse you may select any person, or your estate, to be your Pension beneficiary.
Please ensure that you complete this section of the card.
If you are new to the Plan, you should also complete the Health & Welfare Enrolment
Card and Beneficiary Designation to enrol in the Health & Welfare Plan.
